Faculty Reserve Request Form - This online form can be used by Faculty to request that Library materials be placed on Reserve.
Faculty Reserve Request Form (Word Document) - This form can be used by Faculty to request that print materials be placed on Reserve. It can be used to submit articles to be placed on E-Reserve or Faculty-member's personal books. Materials to be placed on E-Reserves must include the complete citation to the article on the first page. Faculty are responsible for complying with copyright.
